Health Care Worker Vaccine Requirement-State Public Health Officer Order effective September 30, 2021

Health Care Workers will be required to show proof of receiving the COVID 19 Vaccine or submit an exemption for medical and non-medical reasons effective September 30, 2021. 

If you are requesting a non-medical exemption for the COVID 19 vaccine for religious or other philosophical beliefs, you will not be required to obtain a note from a medical provider. You will be required to sign a declination statement. Please check with your employer for requirements on submitting your non-medical exemption form. Non-medical exemptions may require that you are tested for COVID 19 up to twice weekly. In some cases, non-medical exemptions may not be approved by your employer, therefore, it is important to check with your employer on guidelines for the industry in which you work.
